Antibiotics could be promising treatment for form of dementia

Researchers at the University of Kentucky’s College of Medicine have found that a class of antibiotics called aminoglycosides could be a promising treatment for frontotemporal dementia.

Results of their proof of concept study, which was a collaborative effort between UK’s Department of Molecular and Cellular Biochemistry and the University of California San Francisco’s Department of Pathology, were recently published in the journal, Human Molecular Genetics.

Frontotemporal dementia is the most common type of early onset dementia. It typically begins between ages 40 and 65 and affects the frontal and temporal lobes of the brain, which leads to behavior changes, difficulty speaking and writing, and memory deterioration.

A subgroup of patients with frontotemporal dementia have a specific genetic mutation that prevents brain cells from making a protein called progranulin. Although progranulin is not widely understood, its absence is linked to the disease.

A group led by Haining Zhu, a professor in UK’s Department of Molecular and Cellular Biochemistry, discovered that after aminoglycoside antibiotics were added to neuronal cells with this mutation, the cells started making the full-length progranulin protein by skipping the mutation.

“These patients’ brain cells have a mutation that prevents progranulin from being made. The team found that by adding a small antibiotic molecule to the cells, they could ‘trick’ the cellular machinery into making it,” said Matthew Gentry, a co-author of the study and the Antonio S. Turco Endowed Professor in the Department of Molecular and Cellular Biochemistry.

The researchers found two specific aminoglycoside antibiotics – Gentamicin and G418 – were both effective in fixing the mutation and making the functional progranulin protein. After adding Gentamicin or G418 molecules to the affected cells, the progranulin protein level was recovered up to about 50 to 60%.

These results could be promising to drug development. Currently, there are no effective therapies for any type of dementia.

After this preclinical proof of concept study, the next step is to study the antibiotics’ effects on mice with the mutation that causes frontotemporal dementia, Zhu says. Another focus is to possibly develop new compounds from Gentamicin and G418 that could be safer and more effective. Although Gentamicin is an FDA-approved medication, its clinical usage is limited as it is associated with a number of adverse side effects.

“If we can get the right resources and physician to work with, we could potentially repurpose this drug. This is an early stage of the study, but it provides an important proof of concept that these aminoglycoside antibiotics or their derivatives can be a therapeutic avenue for frontotemporal dementia,” said Zhu.

Frontotemporal dementia (FTD) is an early onset dementia and is characterized by progressive atrophy of the frontal and/or temporal lobes. FTD is highly heritable with mutations in progranulin accounting for 5-26% of cases in different populations. Progranulin is involved in endocytosis, secretion and lysosomal processes, but its function under physiological and pathological conditions remains to be defined. Many FTD-causing nonsense progranulin mutations contain a premature termination codon (PTC), thus progranulin haploinsufficiency has been proposed as a major disease mechanism. Currently, there is no effective FTD treatment or therapy.

Heavy Metal Detoxification Could Aid In Treatment of Chronic Lyme Disease

Sufferers of chronic Lyme disease often have a defective immune response when taking the antibiotics needed to help cure them of the disease. This lowered immune system response can be caused by the Lyme disease itself, or by other internal problems such as heavy metal toxicity. The role of heavy metal detoxification in the treatment of chronic Lyme disease has been a point of study for many experts. Some have posited that heavy metal toxicity and Lyme disease go hand-in-hand, and that heavy metal detoxification could help in the long-term treatment of the chronic illness.

Lyme disease is a tick-borne illness that can cause the infected person to experience a wide array of symptoms such as numbness in the limbs, memory loss, dissipated cognitive function, flu-like symptoms and chronic headaches. Antibiotics are generally used to treat both short and long-term Lyme disease, but due to the attack on the body, sufferers also have weakened immune systems that can lead to antibiotics having less efficacy.

Although more research needs to be done on certain stressors and their correlation with antibiotic resistance in Lyme disease patients, promise has been shown in heavy metal detoxification therapy.

Heavy metal toxicity and Lyme disease

Heavy metal toxicity is the build-up of certain heavy metals in the body that can cause a range of problems such as chronic fatigue, depression and other mental health issues, digestive issues and altered cognitive function. It’s also worse in people who have the MTHFR (Methylene tetrahydrofolate reductase) gene mutation, as this mutation leads to problems with the body’s natural methylation process. The methylation process helps keep things functioning at an optimal level by removing certain stressors such as bacteria, pollutants and heavy metals.

When heavy metal toxicity is present in a person suffering from Lyme disease and the MTHFR gene, the treatment of Lyme disease becomes that much more difficult. To help the body process antibiotics, heavy metal detoxification could play a vital role in helping to smooth things along.

The first and simplest way is through diet. What a person eats has a lot to do with how the body functions, and when certain toxins are present in large amounts, as with the case of heavy metal toxicity, the body does not function at the optimal level. Adding foods rich in vitamins and minerals will encourage heavy metal detoxification throughout the body. Good foods to eat are organic fruits and vegetables such as wild blueberries, tomatoes and lemons; cilantro and garlic; and green tea. To avoid increasing ingestion of heavy metals, it’s good to avoid certain foods such as fish, alcohol and brown rice (because of its high levels of arsenic).

Chelation therapy for Lyme patients

Another process used to help rid the body of its overload of heavy metals is chelation therapy. Chelation is a type of chemical process used by doctors for patients whose heavy metal toxicity is too severe to be tackled by diet alone. The process involves the medication ethylenediaminetetraacetic acid (EDTA). EDTA is given intravenously for direct contact with the bloodstream.

When the drug enters the bloodstream, it finds the minerals in the body that attribute to the overall heavy metal detoxification and attaches to them. Once the EDTA has attached itself to the heavy metals it sought out throughout the body, it then begins the process of detox. The compound created by EDTA’s attachment to the minerals is then passed through the body, eventually leaving through the urination process. This is especially important for sufferers of Lyme disease because it contributes to the body’s overall absorption and use of the antibiotics needed for treatment.

Hyperthermia therapy for Lyme patients

As understanding of alternative treatments for Lyme disease is low, patients recovering from the disease are often solely reliant on the strength and efficacy of the antibiotic course they take. That is why their body needs to be running at its best for the antibiotics to work as they should. Hyperthermia is a process used to help lower antibiotic resistance, thus making the treatment more effective for Lyme patients.

Hyperthermia is the process of heating the body internally until a fever breaks. When a fever is present, white cells in the body work overtime to help combat infection. This actually helps antibiotics attack and kill the Lyme bacteria, because the cells designed to fight the disease and elicit an immune response react much faster than if the body was at its regular temperature.

How to treat chronic Lyme disease

Antibiotics are the most efficient treatment of chronic Lyme disease, but there are a variety of ways to help treat both acute and severe cases in conjunction with medication. Supplements aid in the body’s natural processes so that when a Lyme disease patient takes the antibiotics they need to fight the disease, the body is ready to fight. Things such as Vitamin B6, zinc, magnesium and 5MTHF all aid in the proper function of the body. These supplements can affect the way people with the MTHFR gene combat Lyme disease because those vitamins and minerals can duplicate the processes of a normal MTHFR gene. In sufferers of Lyme disease, this encourages the body to complete its natural methylation process with close to full efficacy.

Another way to help the antibiotics do their job is through the use of infrared saunas while doing an antibiotic course. The infrared sauna helps to sweat out heavy metals and other toxins trapped in the body. The heat also helps quicken the metabolic rate, which can aid in quick digestion (and passing) of certain things that aren’t necessary in optimal health. Some studies even go so far as to suggest that sweating can cause the body to get rid of more toxins than the passing of toxins through urine, but both are effective ways of ridding the body of harmful chemicals that can decrease the efficacy of antibiotics used in the treatment of Lyme disease.

LYME SCI: Can treating mast cells help neurological Lyme disease?

One night years ago, when my daughter was at her sickest, she asked me, “Is the chandelier hanging sideways?” With a curious look on my face, I answered, “No. Why?”

She said everything in the room, including the walls, looked tilted. Like in Whoville, the town in Dr. Seuss’s book “Horton Hears a Who!”

A month later, a prestigious children’s hospital diagnosed her with myalgic encelphalomyelitis (ME) and chronic fatigue syndrome (CFS). The doctors gave no other explanation for her laundry list of symptoms, including a recurring fever.

Little did we know that she had a raging infection inside her brain. Her nightly visits to Whoville continued for months. They finally stopped, after she was diagnosed with neurological Lyme disease and co-infections, and treated with antibiotics.

Patients with late-stage Lyme are no strangers to the neurological manifestations of this illness. Symptoms like brain fog, anxiety, depression and insomnia are common complaints in this population. But what causes all these symptoms? Inflammation. Like in many other illnesses, inflammation is the enemy.

At ILADS 2019, Dr. Tania Dempsey emphasized the importance of diagnosing and treating mast cell activation syndrome when it is suspected in the course of Lyme disease.

Mast cells are a complicated aspect of our immune system. When mast cells go haywire, they cause inflammation. (I’ve written extensively about mast cell diagnosis, treatment and how it helped solve the puzzle of my daughters illness.)

Dr. Dempsey has found that incorporating mast cell treatment into her Lyme protocols helps to reduce many of the symptoms of “Lyme brain” by controlling inflammation. In my daughter’s case, finding the right mast cell protocol was a critical step in getting her into remission.

Disseminated neurological Lyme disease

Neurological Lyme disease symptoms are largely invisible and difficult to quantify. Thus, many physicians write them off as psychosomatic, leaving these patients with no validation or treatment options.

Until recently, there hasn’t been a good measurement for patients with these long-standing neurological symptoms following treatment for Lyme disease.

Two separate studies published in 2019 by renowned researchers—one from Johns Hopkins University, the other from Harvard—have finally given us quantifiable, physiological reasons for these debilitating neurological symptoms.

The Johns Hopkins study revealed chemical changes and widespread inflammation in the brains of patients with persistent symptoms following treatment for Lyme disease. The Harvard study showed nerve damage (small fiber neuropathy) and decreased blood flow in the brains that may also contribute to dysautonomia. (Dysautonomia is a malfunction of the autonomic nervous system. It can cause a variety of troublesome symptoms.)

Left untreated, Lyme disease can spread to the nervous system. There are a multitude of diseases associated with neurologic Lyme:

Lyme meningitis – inflammation of the membranes that cover the brain/spinal cord.
Lyme encephalitis – inflammation inside the brain
Lyme myelopathy – inflammation of the spinal cord
Lyme cranial neuritis – inflammation of the cranial nerves
Lyme neuropathy – inflammation of the peripheral nerves
Bannwarth’s syndrome – “terrible triad” of meningitis, cranial neuritis and painful neuropathy

Nervous system basics

The nervous system has two main parts. The central nervous system consists of the brain and spinal cord. The peripheral nervous system includes all the nerves outside of the central nervous system. Lyme disease can infect either or both.

The central nervous system has a special protective layer called the blood brain barrier (BBB). It lets in nutrients like oxygen and glucose, and keeps out pathogens. Without the BBB, the brain would be open to infection from every germ that enters the blood stream.

A new study from UC Berkeley has found that a leaky BBB leads to memory loss and cognitive dysfunction in older mice. By giving these rodents drugs to reduce inflammation in their brains, the researchers reversed their signs of aging.

“When you remove that inflammatory fog, within days the aged brain acts like a young brain. It is a really, really optimistic finding, in terms of the capacity for plasticity that exists in the brain. We can reverse brain aging,” said one of the researchers, Professor Daniela Kaufer. Their study has profound implications for human brains too.

Mast cells: a gateway to the brain and nervous system

Mast cells play a major role in defending our bodies from pathogens. On the brain side of the blood-brain barrier, they serve as “universal alarm cells” that start the inflammatory cascade.

During infection of the central nervous system, the number of mast cells will increase, contributing to inflammation. During chronic inflammation, the BBB becomes more permeable—leaky– allowing mast cells to travel out of the brain. Unfortunately, the leaky BBB also allows large white blood cells and blood-borne pathogens into the brain.

Mast cells contain many different types of chemicals. The most abundant one is tryptase, a powerful enzyme released during mast cell activity. At ILADS, Dr. Theo Theoharides explained that tryptase acts almost like a “meat tenderizer.” As you can imagine, something that can “tenderize” the nervous system would be pretty destructive.

When mast cells become over-reactive, they create a continuous loop of inflammation that adversely impacts nerves and the nervous system.

In my daughter’s case, the mast cells became activated during her untreated infection. They remained so until we removed triggers, incorporated a low-histamine diet, and added treatment for mast cell activation syndrome (MCAS). After two years of a strict diet and MCAS treatment, it appears her mast cells have partially returned to normal.

Symptoms of neurological Lyme disease

The MyLymeData project asked Lyme patients who were still ill to identify their three worst symptoms. It turns out, the most frequently reported “worst symptoms” were neurological.

The most frequently reported neurological Lyme disease symptoms in MyLymeData included memory loss and cognitive impairment, sleep impairment, psychiatric manifestations, headaches, and neuropathy.

Infections within the CNS are much more difficult to treat, because most oral antibiotics do not cross the BBB. Even with IV ceftriaxone, the drug of choice for neurologic Lyme disease, many patients will be left with chronic symptoms.

While doctors don’t all agree about the cause of these ongoing symptoms, many ILADS doctors believe treatment should continue until symptoms have resolved. Some also report that intravenous immunoglobulin (IVIG) or plasmapheresis can also be helpful in treating infection-induced autoimmune encephalitis.

In addition, as we learned at ILADS this year, many doctors are seeing improved results by incorporating MCAS treatment into their Lyme protocols.

There is hope

As my daughter has shared, you can heal from Lyme and mast cell activation. Jesse Colin Young, Kris Kristofferson, Amy Tan, Yolanda Foster, and many others have reported that they got their brains back after being treated for Lyme disease.

As science sheds new light on disorders of the nervous system, the Lyme community stands to gain. My message to you is, don’t give up hope.

Fantastic article that points out many issues as to why many Lyme/MSIDS patients struggle. MCAS is a big one. Calming the body down is a huge aspect of recovery. In essence, Lyme/MSIDS messes up the immune system and causes wide-spread inflammation. Due to this, many suffer with sensitivities, from food to their environment. The body over reacts to these things as if it’s being attacked, causing a wide variety of symptoms, making life miserable.

Under the new guidance ALL homeopathic medicines will be considered illegal. This is because the Draft Guidance declares all homeopathic medicines to be “new drugs” which have not undergone the agency’s pre-market approval process known as a New Drug Application (NDA). This “new drug” designation makes this new guidance particularly damning for homeopathy. Homeopathic medicines cannot be patented and therefore cannot justify the enormous expenditures that an NDA entails. If the FDA insists that a homeopathic medicine be put through the NDA process, the agency will essentially be issuing a potential ban on that medicine in the United States.

If the Draft Guidance is adopted unchanged ─ and especially if the designation of all homeopathic medicines as “new drugs” is allowed to stand ─ consumers will likely begin to see homeopathic medicines disappear from store shelves and practitioners’ offices as the FDA takes aim at one homeopathic medicine after another.

Regardless of your opinion of homeopathy, I find it interesting that the only things the FDA seems capable of regulating are competitors to Big Pharma. Compounding pharmacies have the strictest regulations on earth and now it appears they want to screw the bolts into homeopathy as well.
